Leadership Council General Release & Registration Sweet Readers, Inc. (”The Charity”) is a 501(c)3 tax exempt public charity, founded in 2011 in New York and based in New York and Massachusetts. Our mission is to empower young people through training, transformative programs and communities of support to engage isolated older adults living with and without Alzheimer’s and related disorders (AD) and become catalysts for excellence in eldercare. During Sweet Readers programs, young people are challenged to utilize science, human engagement and the arts to discover the person behind the disease and meaningfully connect. We believe the bonds that are formed help to build empathic communities where everyone is seen, heard and valued. The Sweet Readers Leadership Council’s (the “Council”) was established in 2015 by three high school students, Sweet Readers Alumni and the Sweet Readers Co-Founder, Sophie Young. Their mission is to develop and support Sweet Readers communities and educate the public about AD, isolation and brain health. To date, the Council has raised awareness about Alzheimer’s, isolation and brain health in all 50 states and 70 countries through their grassroots efforts and documentaries and is responsible for developing and sustaining Sweet Readers communities in Canada, London and Chicago and helping to launch Sweet Readers en Español! We are delighted to welcome your child (my “Child”) to our Council as a Young Leader and appreciate your support! Young Leaders participate in a training and team building retreat (the “Retreat”, September 18-19, 2021 in New York, New York), develop curriculums, help train Sweet Readers, engage new and existing partners (eldercare centers, schools, museums and/or stakeholders, the “Partners”) for and of The Charity and participate in and/or facilitate Sweet Readers multigenerational programs. They also participate in small and large group meetings throughout the school year and two community events: The Next Steps Gathering, to be held on November 11, 2021, during which they present to friends and family their plans for the year ahead, and the Insights & Appreciation Gathering which, for 2022 will be replaced by our special 11th Anniversary Celebration on May 2, 2022. Finally, Young Leaders help to manage and participate in The Charity’s communications, including but not limited to, the Podcast, Newsletter, Social Media Platforms and Web site. For purposes of this Agreement, the above referenced Retreat, trainings, meetings, programs, communications and special events will collectively be called the “Programs”. The Council’s Retreat will be headquartered at 1107 Fifth Avenue, in the ground floor apartment located on East 92nd Street in Manhattan (“Headquarters”). During the Retreat, Young Leaders will participate in leadership and team building skills activities to be held in the Headquarters, in Central Park and in major museums within an approximately one mile radius of the Headquarters. The Council Handbook outlines the Charity’s and Council’s missions, ethos and Methods as well as the important protocols to ensure a safe, impactful and enriching experience for all. Young Leaders will receive a digital copy of the Handbook prior to the start of the Council’s September Retreat. I acknowledge, understand and agree, as a parent or legal guardian for the Young Leader listed below, to each of the following terms and provisions: 1. My Child is to the best of my knowledge, in good health to undertake the normal range of activities contemplated by the Programs. I also acknowledge that as a condition of participating in the Programs, my Child possesses comprehensive medical/health insurance which will be in effect throughout my Child's participation in the Programs and my Child will be fully vaccinated and adhere to the COVID19 protocols as set forth and updated by The Charity, presented in the Handbook and also updated via email to families as the requirements evolve; and 2. My Child will be participating fully in the following three mandatory events and at least one parent or guardian will participate in honor of my child, in both the Next Steps Gathering and the Sweet Readers 11th Anniversary Celebration: The Retreat: September 18-19 in New York City. The Next Steps Gathering: November 11, 2021. Remote, approximately 6:15 pm - 7:45 pm Eastern Standard Time. The Sweet Readers 11th Anniversary Celebration: May 2, 2022 both in person in New York City and streamed remotely outside of the tri-state (New York, New Jersey and Connecticut) area. Approximately 6:00 pm - 8:00 pm Eastern Standard Time. 3. THE RETREAT: a. I am responsible for all travel and lodging expenses incurred in order for my Child to participate fully; with the exception that The Charity will cover the cost of and will help expedite transportation to and from the New York airport, at my request, if my Child requires air travel to attend. b. The Charity offers financial aid to cover travel expenses on a needs basis and will provide complimentary housing options for any child who is traveling from out of the country, with a family of a fellow Young Leader who lives in New York City, upon request. c. Any other expenses incurred by my Child during the course of the Retreat will be my sole responsibility, except that the Charity will provide and cover the cost of breakfast, lunch and dinner on September 18, breakfast and lunch on September 19 along with healthy snacks for both days, museum admission fees and all program materials required throughout the Retreat. d. My Child must email to Samantha@sweetreaders at least five days prior to the start of the Retreat, a copy of proof of their being fully vaccinated and carry this proof with them at all times throughout the Retreat. e. There will be at least one adult chaperone present throughout the retreat, serving in a supervisory capacity for all workshops, activities and events, except for a special “Museum Race” where small teams of Young Leaders will travel on their own throughout the Metropolitan Museum of Art as part of a creative exercise. My Child will also have a few breaks, without adult supervision, during the course of the Retreat where they will be free to travel within a one mile radius of the Headquarters. There will be no adult supervision provided outside of the Retreat’s hours which are: Saturday, September 18: 8:00 am – 8:00 pm and Sunday, September 19 from 8:00 am – 4:30 pm. f. My Child will bring his/her/their yoga mat, if they have one and face masks to the retreat and will notify the Charity (by emailing Samantha@sweetreaders.org) within five days prior to the start of the Retreat if they do not have their own yoga mat and/or face masks and in that case, the Charity will provide a yoga mat and face masks for them. g. Prior to the Retreat, The Charity will email me and my Child an updated list of all protocols for conduct and health and safety requirements within the Headquarters, in Central Park and in any other places associated (ie. Museums, restaurants and/or private homes) with the Retreat and I agree to review these protocols with my Child to help ensure my Child understands and adheres to them fully. h. My Child is required to abide by all applicable local, state and federal laws, rules and regulations throughout the Retreat, just as my Child would be obligated and required to comply with any such applicable laws, rules and regulations in the absence of the Retreat or The Charity. 4. In the interest of consistency and to give my Child the best opportunity to have meaningful impact, I will make best efforts to ensure my Child attends every session in both the mandatory and elective Programs to which he/she/they has committed. If, in the event of an emergency, my Child is unable to attend a session, I will encourage my Child to notify the appropriate person(s) as outlined in the Council’s Handbook as soon as practicably possible. 5. As a Young Leader, my Child remains fully subject to The Charity’s and the Partner's protocols concerning my Child's conduct, including the right of the Charity to suspend, limit or terminate my Child's participation in any of the Programs or the Council as a whole, for any reason the Charity deems proper. 6. As part of my Child's participation in the Programs, my Child may be asked to fill out evaluations to support the Charity's ongoing efforts to improve the quality of their programming. These evaluations may also become part of a larger study of the impact of the Programs on participants. My Child agrees to answer the questions to the best of his/her/their knowledge and the Charity agrees not to publish my Child's last name in connection with the Program without my prior written consent. Leadership Council General Release & Registration Image, Voice & Works Release I hereby irrevocably consent to and authorize the Charity to use any photographs, audio and/or videos of my Child and/or any works (artworks, poetry, music, curriculums, etc.) my child may create during the course of the Program in order to promote and publicize the Charity, its mission and its programs, in any medium. The Charity agrees not to publish my Child's last name without my prior written permission. I further acknowledge that The Charity is the owner of all rights in and to the aforementioned photographs, videos and/or works. By signing below I agree to allow my Child to participate in the Programs and hereby release and will indemnify and hold harmless The Charity, its employees, officers, directors, agents and representatives, to the fullest extent permitted by applicable law, from all claims, demands and liability of any kind including without limitation, all personal injuries including loss of life and expenses including reasonable attorney's fees, arising my Child's participation in the Programs and/or the display, distribution, reproduction or other use of the photos, videos, audio and/or works which may be produced during or for the Programs, excluding claims arising directly from the willful misconduct or grossly negligent acts of the Chairty. The indemnity set forth in this paragraph shall survive termination of this Release.
